The Blooming Bridges Foundation Notice of Privacy Practices

As required by the Privacy Regulations created as a result of the Health Insurance Portability and Accountability Act of 1996 (HIPAA),

THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU (AS A CLIENT OR REPRESENTATIVE OF THIS SERVICE) MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO YOUR INDIVIDUALLY IDENTIFIABLE HEALTH INFORMATION.

Our Commitment to Your Privacy

The Blooming Bridges Foundation is dedicated to maintaining the privacy of your protected health information (PHI). In conducting our business, we will create records regarding you and the treatment and services we provide to you. We're required by law to maintain the confidentiality of health information that identifies you. We're also required by law to provide you with this notice of our legal duties and the privacy practices that we maintain in our practice concerning your PHI. By federal and state law, we must follow the terms of the notice of privacy practices that we have in effect at the time of your care.

  • Request confidential communication
  • Ask us to limit the information we share
  • Receive a list of those with whom we have shared your information
  • Receive a copy of this privacy notice
  • Choose someone to act for you
  • File a complaint if you believe your privacy rights have been violated

How We May Use and Share Your Information

We may use and share your information as we:

  • Provide you with services
  • Bill for services we provided
  • Do research
  • Comply with the law
  • Address workers’ compensation, law enforcement, and other government requests
  • Respond to lawsuits and legal actions

Filing a Complaint If You Feel Your Rights Are Violated
